APPOINTMENTS - I will make every effort to respect your scheduled time but occasionally may be delayed by emergencies. Please be aware that if you are late, your appointment may need to be
shorter, or we may need to reschedule it out of respect for the next patient. As of 3/2020 I will provide tele psychiatry appointments exclusively by phone or at doxy.me/drmattia during
your scheduled time.
REFILLS/PHONE CALLS - Please request refills at your appointment whenever possible. I will do my best to respond within 24 hours to refill requests and calls between appointments.
Requests made on Fridays may not be addressed until the following MOnday.
NO SHOW FEE - I appreciate 24 hours advance notice if you are unable to attend your appointment. This allows others to have access to care. Although I understand there are circumstances that
interfere with your attendance., I reserve the right to charge a $75 no show fee if appointments are not cancelled at least one business day in advance.
PAYMENT- I will submit billing information to your insurance company. Please check with your insurance company to see what services are covered by your policy and bring your insurance card
with you to each visit. I accept credit cards, checks, and cash.
EMERGENCIES - Please call 911, the Crisis Center (436-8888), or Bellin Psychiatric Center for inpatient assessment. Routine messages may be left on my voicemail during regular business
hours 920-965-6768.
